We prefer to see patients on an appointment basis. We understand the importance of time and make every effort to honor all time commitments; we request that you do the same for us. On occasion, emergencies may arise. You will be informed if there will be a delay in your scheduled visit.

Your appointment is scheduled only for you. We request that you make every effort to keep your scheduled appointment.

Directions and Map to our Office
We are located in Village Green Shopping Center which is just northwest of the intersection of Greenwood Avenue and Northwest Highway in Park Ridge. Our office is in the middle of the shopping center, across the courtyard from Blockbuster.

From Chicago: Take the Kennedy Expressway (I-90) west, exit at Cumberland Avenue north. Proceed north on Cumberland Avenue and follow the map.

From the South: Tri-State Tollway (I-294) north, exit Touhy Avenue east, then north on Greenwood and follow the map.

From the North: Tri-State Tollway (I-294) south to the Kennedy Expressway (I-90) east. Exit on Cumberland Avenue north and follow the map.

From the West: Northwest Tollway (I-90) east, becomes Kennedy Expressway, exit Cumberland Avenue north and follow the map.
